My credit card company is investigating.  I will post the results just in case this happens to someone else. 

Can anyone tell me a website where I can see if the Penalty Code even exists?

They said they do not have them. Only charged us to provide them our car rental information.  I will try a different agent, maybe more helpful.  Thank you for your suggestion.
