Marksfish, it's quite simple really.
The ADSL router RJ11 connector goes directly into a Telefonica wall socket (also RJ11). Then all other telephone devices like phones, fax machines or computer modems need to be connected through a microfilter to avoid them interfering with the ADSL signal. In effect these other devices are split from the main telephone line by the filters.
Telefonica microfilters are simple devices with a single RJ11 plug on a short lead at one end, and an RJ11 socket to plug the phone etc. into. If you need to plug the ADSL router and a phone into a single wall socket, you use a small two-way adaptor to plug the two RJ11 plugs into a single wall socket.
ADSL routers supplied by Telefonica come with three microfilters (the maximum you are able to connect on a single line) and you put them on everything connected to the line, apart from the router itself. The ADSL router is always connected directly and unfiltered, whilst all other devices are filtered.
