Lemon Tree

My lemon tree has been ill since I moved in 4 years ago. I've tried to make it better but with no luck. It has hardly any leaves those which it has are yellow with brown spots, hardly any fruit, bleached spots on the bark. I can't see any bugs. Has anyone any idea whats wrong with it ?
