Declaration of tax residency forms?

 Hi, 

I wonder if anyone can help me? I am working my way through a backlog of paperwork and I've  come across some' Declaracion de Residencia Fiscal' forms in Spanish, I have lost the accompanying paperwork and I can't remember where they came from. Does anyone know where I should send these forms once I've completed them?  I normally get an English company to pay my Spanish Tax every year so haven't dealt with the taxes before.

This is normally sent to you by your bank - ours has just arrived.

A form has to be completed every 2 years to ensure that you do not pay taxes as a non-resident.

Our form has the banks address at the bottom to which we should send the completed form
